ABSTRACT
Tuberculosis
is an infectious disease which remains a public health problem and one of the
death causes necessarily need prevention programs to be implemented on an
ongoing basis. Nevertheless, by the development of the times and the emergence
of other problems related to TB in Indonesia at this time is the increasing
cases of MDR-TB (Multi Drug Resistant). MDR-TB control transactions are carried
out by utilizing Strategy "DOTS PLUS" with the oncoming program of
Integrated Management of Drug Resistant TB Control (MTPTRO). Puskesmas Helvetia
is one of the first-level health facilities which had been done prevention
program of MDR-TB in the city of Medan. MDR-TB patients were recorded since the
year 2010-2015 as many as 8 people who was including 2 deaths, 3 people missing
(defaulters) and 3 are still in the running treatment.
This
study was a qualitative research aimed to identify clearly and more deeply
about the management of MDR-TB control programs in Puskesmas Helvetia. Methods
of data collection was taken by in-depth interviews and observations of 9
informants consisting of staff of Medan City Health Department, Head of
Puskesmas Helvetia, TB Officer, 3 people with MDR-TB, and 3 PMO. The data was
analyzed by the method of Miles and Huberman.
The
results indicated the treatment of MDR-TB control programs in Puskesmas
Helvetia had not been done optimally. It was seen from the quality of personnel
in case finding carried passively by waiting patients who came to the clinic,
the lack of available facilities and infrastructure support, the absence of
monitoring results of treatment provided, as well as the lack of counseling
given by the clerk of TB patients, PMO and society.
Based on the results of the study, it was
expected to Medan City Health Department in order to provide the allocation of
funds and improve the performance of health workers, to the officer of TB they
need to be more active in inventing the cases, to provide counseling and
monitoring of treatment outcomes for patients with MDR-TB, PMO, and the
community.
